It is our mission at UHD to support the best possible care of each patient that works through our doors. We constantly strive to provide better patient care by working with our medical teams to create a healing environment that is of value to all of our patients. In general, the basics of a discharge plan are:
  • Evaluation of the patient
  • Discussion with the patient of his/her representation
  • Planning for homecoming or transfer to another care facility
  • Determining if caregiver training or other support is needed
  • Referrals to home care agency and/or other appropriate community support organizations
  • Arranging for follow-up appointments or tests

Effective discharge planning can decrease the chances that you or your family member is readmitted to the hospital. Research has shown that excellent planning and good follow-up can improve patients’ health, reduce re-admissions and decrease healthcare costs.
