Social Services
The role of our Social Services department is to help United Hospital District patients plan and access services related to their healthcare needs. Whether you are transitioning from a hospital stay, living with chronic illness or need assistance with age-related issues, our goal is to support your healthcare journey.
UHD’s Social Services team can help with coordinating referrals and identifying resources for many services, including:
- Advance directives
- Extended care services (assisted living, skilled nursing facilities, nursing homes)
- Financial assistance eligibility programs (Medicare, Medicaid and others)
- Home health care
- Home medical equipment
- Hospice care
- Meals on Wheels
- Senior services (lifeline, caregiver, senior centers)
- Translator services
- Transportation to and from medical appointments
